The March 1, 2010 drawing for a 32gb iPod Touch ($300 value) is offered by Elite Sports Physical Therapy (ESPT).

Entry forms are given to Entrants for each new patient they refer to Elite Sports Physical Therapy

Entrants need not be present at drawing to win.

Entrants are allowed to be employees of ESPT or their family members, with the exception of Ikaika Health, Inc. officers or their family members.

To enter the ESPT iPod Drawing, Entrants can:

1. Ask the Referral Patient to bring in the postcard that has been addressed to the Entrant, or

2. Complete this drawing entry form (PDF) and ask the Referral Patient to bring it in to their appointment.

Referral Patient must have qualifying insurance plans or be willing to pay cash- pay rates to allow for physical therapy treatment at ESPT

Free consultations do not constitute qualified Referral Patients.

All entries must be received by 7:00pm on February 28, 2010. The drawing will occur the following day, March 1, 2010, and the Winner will be notified by either phone and/or email.

No responsibility will be assumed by ESPT for lost, misdirected, or late entry forms. Entrants may contact us at any time for details about their entries and a receipt of entry can be provided if so desired. It is the sole responsibility of the Entrant to ensure their Referral Patient notifies ESPT appropriately to enter them in the drawing.

Winner will be notified by phone and/or email and results will be posted on ESPT’s website. Entry constitutes permission to use the winner’s name and likeness for publicity or promotional purposes without additional compensation, except where prohibited by law.

Taxes, licenses, and fees are the responsibility of the winner. The winner will be subject to any tax laws that apply. All such taxes shall be paid by the winner according to the Internal Revenue Service rules and regulations.

Odds of winning vary and depend on the number of entries received.

All persons submitting an entry agree to the contest rules and regulations set forth above. ESPT reserves the right to make minor revisions to contest rules as deemed appropriate.
